    A provenance task abstraction framework

    Visual analytics tools integrate provenance recording to externalize analytic processes or user insights. Provenance can be captured on varying levels of detail, and in turn activities can be characterized from different granularities. However, current approaches do not support inferring activities that can only be characterized across multiple levels of provenance. We propose a task abstraction framework that consists of a three stage approach, composed of (1) initializing a provenance task hierarchy, (2) parsing the provenance hierarchy by using an abstraction mapping mechanism, and (3) leveraging the task hierarchy in an analytical tool. Furthermore, we identify implications to accommodate iterative refinement, context, variability, and uncertainty during all stages of the framework. A use case describes exemplifies our abstraction framework, demonstrating how context can influence the provenance hierarchy to support analysis. The paper concludes with an agenda, raising and discussing challenges that need to be considered for successfully implementing such a framework

    Design and rationale of a 16-week adjunctive randomized placebo-controlled trial of mitochondrial agents for the treatment of bipolar depression

    Objective: Bipolar disorder places a significant burden on individuals, caregivers and family, and the broader community. Current treatments are believed to be more effective against manic symptoms, leaving a shortfall in recovery during the depressive phase of the illness. The current study draws on recent evidence suggesting that, in addition to increased oxidative load, alterations in mitochondrial function occur in bipolar disorder. Methods: This 16-week study aims to explore the potential benefits of N-acetylcysteine (NAC) alone or in combination (CT) with selected nutraceuticals believed to enhance mitochondrial function. The study includes adults diagnosed with bipolar disorder currently experiencing an episode of depression. Participants are asked to take NAC, CT, or placebo in addition to any usual treatments. A post-discontinuation visit is conducted 4 weeks following the treatment phase. Results: The primary outcome of the study will be mean change on the Montgomery-Asberg Depression Rating Scale. Secondary outcomes include functioning, substance use, mania ratings, and quality of life. Blood samples will be collected at baseline and week 16 to explore biochemical alterations following treatment. Conclusion: This study may provide a novel adjunctive treatment for bipolar depression. Analysis of biological samples may assist in understanding the therapeutic benefits and the underlying etiology of bipolar depression

    Multilingual construction of Communicative Development Inventories in Southern Africa

    The Communicative Development Inventories (CDIs) are a set of validated, parent-completed questionnaires assessing children's vocabulary, gesture, and grammatical development. They have many benefits but in particular administration does not require a qualified psychologist or speech-language therapist, making them ideal for settings with poor access to professionals. In addition they are cheap to administer at scale, again benefitting low resource settings. Further, they are relatively easy to develop for languages that are not well described in terms of development, meaning they can be adapted easily to under-studied languages. The CDIs have previously been adapted for Asian (1), Indo-European (2), other European (3), Pacific Island (4) and some African languages (5,6). They have been used successfully in studies of health risks (nutrition, infection) in sub-Saharan Africa (7), again meaning they are useful for impoverished settings and child development issues prevalent in such settings. We are in process of developing CDIs for six languages spoken in Southern Africa (IsiXhosa, Setswana, Sesotho, Xitsonga, Afrikaans and South African English). We have developed a common method for each language starting with a long list of words taken from other CDIs, following on to parent/practitioner assessment of face validity of these words in their translations, and continuing to piloting, instrument reduction, and validation. Within validation we look at correlation with age, and relationship to other variables including family socio-economic status, and child language performance measured in the home or lab. We also apply these methods to the assessment of gesture using the CDI. Some of the languages to be studied are related to each other (of the Bantu group of African languages) and to two languages for which CDIs have been developed in East Africa (5); for these we will highlight common grammatical development to be examined that exists in all of the related languages. In the East African setting parents were found to be valid observers of their child's morpheme omissions so this is a particular focus for assessment of grammatical development. To date we have developed four pilot versions (IsiXhosa, Setswana, Sesotho, and Afrikaans). Vocabulary in isiXhosa is found to be significantly related to toddler age, mother education, and first-born status, among other known correlates of child language (8). In Sesotho, toddler vocabulary is also significantly related to age (9). In the other languages (Xitsonga and SA English) we have created pre-pilot word lists and carried out focus groups with parents enabling us to determine the acceptability of the words on our long-lists. We conclude that construction of CDIs in multiple (some unrelated) languages simultaneously is possible with rigorous application of the same methods to each language.     A retrospective analysis of survival and prognostic factors after stereotactic radiosurgery for aggressive meningiomas

    BACKGROUND: While most meningiomas are benign, aggressive meningiomas are associated with high levels of recurrence and mortality. A single institution’s Gamma Knife radiosurgical experience with atypical and malignant meningiomas is presented, stratified by the most recent WHO classification. METHODS: Thirty-one patients with atypical and 4 patients with malignant meningiomas treated with Gamma Knife radiosurgery between July 2000 and July 2011 were retrospectively reviewed. All patients underwent prior surgical resection. Overall survival was the primary endpoint and rate of disease recurrence in the brain was a secondary endpoint. Patients who had previous radiotherapy or prior surgical resection were included. Kaplan-Meier and Cox proportional hazards models were used to estimate survival and identify factors predictive of recurrence and survival. RESULTS: Post-Gamma Knife recurrence was identified in 11 patients (31.4%) with a median overall survival of 36 months and progression-free survival of 25.8 months. Nine patients (25.7%) had died. Three-year overall survival (OS) and progression-free survival (PFS) rates were 78.0% and 65.0%, respectively. WHO grade II 3-year OS and PFS were 83.4% and 70.1%, while WHO grade III 3-year OS and PFS were 33.3% and 0%. Recurrence rate was significantly higher in patients with a prior history of benign meningioma, nuclear atypia, high mitotic rate, spontaneous necrosis, and WHO grade III diagnosis on univariate analysis; only WHO grade III diagnosis was significant on multivariate analysis. Overall survival was adversely affected in patients with WHO grade III diagnosis, prior history of benign meningioma, prior fractionated radiotherapy, larger tumor volume, and higher isocenter number on univariate analysis; WHO grade III diagnosis and larger treated tumor volume were significant on multivariate analysis. CONCLUSION: Atypical and anaplastic meningiomas remain difficult tumors to treat. WHO grade III diagnosis and treated tumor volume were significantly predictive of recurrence and survival on multivariate analysis in aggressive meningioma patients treated with radiosurgery. Larger tumor size predicts poor survival, while nuclear atypia, necrosis, and increased mitotic rate are risk factors for recurrence. Clinical and pathologic predictors may help identify patients that are at higher risk for recurrence
